The PYLUSD
Reflection Competition and Contest?
DEAD LINE extended to Oct. 15th
Any student from grades K-6 can enter the Competition
Nine (9) winners will be chosen from Glenview
in the areas of Art, Photography, Literature, etc.
Entry forms are in the front office.
The deadline is October 15th.
Winners will be represented at the District office
and will have a party!
For the contest, think of a new theme for
the 2012 Reflections Program
The winner gets $100 and their theme will be represented in 2012!

BIRTH CERTIFICATE:
Original birth certificate, and your child must be 5 years old on or before December 2, 2008
IMMUNIZATION RECORD:
Must be up-to-date and include month, day, year for Polio, DPY, Measles-Mumps-Rubella, and Hepatitis B. A dose of Varicell (chicken pox) is required or disease documented by health care provider.
PHYSICAL EXAM:
A physical exam is required AFTER March 1 of Kindergarten entry year UP to December 30 of First Grade. (18 months is allowed to complete this requirement; however it is suggested that you complete the physical as soon as possible.
PROOF OF RESIDENCY:
Current GAS or Electric bill, rental or lease agreement, or escrow papers.
